New patients: You will be applying to the practice and requesting an appointment here. We will get back to you as soon as possible if your appointment has been accepted. Steps to follow:

2. Select your desired appointment type.
3. Enter your information. During the booking, you will be asked to place a credit card on hold.
4. Please note we only accept Traditional Medicare Part B.
5. Upload a copy of your driver's license or other ID, as well as any records you would like Dr. Chalfin to skim before your visit. Most other records can be reviewed at the time of your appointment.
