IMPORTANT NOTE ABOUT COVID-19 CORONAVIRUS: Most patients will still be seen via telemedicine (video) consultations instead of in-person. Later, as Dr. Chalfin deems necessary, she will schedule you in person. At this time, insurance companies we are in-network with (Medicare and Medicare supplement plans ONLY) are covering telemedicine visits the same as regular visits. If you have any questions, please call your insurance company.
New patients: You will be requesting an appointment here. We will get back to you within the next week either confirming or declining your requested appointment.
Steps for Requesting Your Appointment
- Review our Office Policies and Frequently Asked Questions
- Select your desired appointment type (New or Established Patient).
- Enter your information. During the booking, you will be asked to place a credit card on hold.
- Accepted insurances: Traditional Medicare only. As secondary plans for Medicare, we accept only Medigap (official Medicare supplement) plans. If you have any other commercial or employer-provided health insurance plan, Dr. Chalfin is most likely out of network.
- Upload a copy of your driver's license or other ID, and the front and back of your health insurance card, as well as any records.
- Expect an email with important forms to fill out.
